

The Transitional Programme bridges the gap between the structure and routine of the main neuro-rehabilitation programme and community living. It is also useful for clients who have a brain injury and need to re-learn skills to help them live more independently within a local community.
A client-centred, goal-orientated approach is used. Following an assessment period, and taking into account the client’s aspirations for the future, an action plan is created that builds on their strengths and addresses their needs using a graded rehabilitative approach and a neurological frame of reference. Occupational Therapy Technical Instructors and Support Assistants implement guidelines and strategies, supported and aided by the Occupational Therapy and the Psychology Departments.
Clients are able to access therapies and formal education sessions, if appropriate, to their needs. These sessions are treated as ‘appointments’ and clients are supported to meet them and plan their days around them.
The Transitional Unit has five bedrooms. It is most suitable for clients who need support to learn how to use strategies and skills within functional everyday activities; activities that are relevant to their future lives within the wider community. The unit is also suitable for clients who are able to learn slowly or require a slower pace due to slow information processing. Clients who are likely to live in a Group Home situation can also benefit from the Transitional Unit’s programme.
The programme includes set times for some functional tasks, such as shopping, cleaning, budgeting and planning. At other times clients learn skills to manage their unstructured time. For example, this time can be used to pursue hobbies and/or to learn new ones, to plan outings, to create and produce memory lists, to work on recipe files and leisure resources and use of community resources.
